HOMEBOUND SERVICES

The faculty and staff of ACS desires to provide continuity in a student's education by providing modified instruction due to illness or injury. Homebound instruction offers temporary educational services to students who are unable to attend school for longer than ten (10) consecutive days due to illness or injury.

INITIAL CERTIFICATION PROCESS

In order to determine a student's eligibility for homebound services, the parent/guardian must complete and submit the following forms to Dr. Keri Prigmore at the Educator Support Center:

  1. Release of Records
  2. Initial Certification
The medical documentation section MUST be completed and signed by the attending licensed specialist.  Forms signed by non-licensed specialist, such as physician assistants, nurses, nurse practitioners etc., WILL NOT be accepted.
If a student's condition requires homebound services for a period longer then thirty (30) school days, the homebound liaison will submit a recertification form prior to the end of the homebound period.
