Enrollment
Please provide the following for proof of residence:
- Signed copy of your lease, mortgage statement, or deed
- Current utility (electric, gas, or water) bill or service set-up agreement. The mailing and service address must match and be visible.
If the guardian's name is not on the documents mentioned above, this is considered a shared residence situation, and we will need the following:
From the homeowner/lessee:
- Signed copy of your lease, mortgage statement, or deed
- Current utility bill/service agreement
From the Guardian:
- Government-issued identification and one significant piece of mail, each of which must bear the address of the homeowner/lessee
- If the guardian is unable to obtain government-issued identification, you must provide two significant pieces of mail bearing the address of the homeowner/lessee.
